Job summary

Lumia Care is seeking a Service Coordinator to join the Tuggerah team. You will support Care Partners by coordinating client plans, managing services against approved budgets and liaising with providers to ensure delivery of supports.

The role emphasizes organisational efficiency, attention to detail and strong communication across multiple teams and systems within a busy community services environment.

Qualifications

• Experience within Support at Home, Home Care Packages, NDIS or disability services.
• Strong administration and coordination skills.
• Excellent attention to detail.
• Confidence working across different systems.
• Good understanding of plans, budgets and service coordination.
• Strong communication skills.

Responsibilities

• Supporting Care Partners with the administration and coordination of client plans
• Managing services against approved plans and budgets
• Coordinating services and providers
• Maintaining accurate client records and documentation
• Monitoring plan utilisation and identifying issues
• Following up actions and ensuring services are delivered as planned
• Liaising with internal teams, clients and external providers
• Taking ownership of the administrative detail that keeps client services running smoothly
