Job summary

Lumia Care is recruiting a Service Coordinator to join their Hawthorn team, supporting Care Partners with administering client plans and coordinating services behind the scenes of Support at Home.

Ideal candidates will have CHSP or NDIS plan management knowledge, be highly organised, meticulous, and comfortable navigating multiple systems while keeping documentation accurate and on track for service delivery.

Qualifications

• Experience in Support at Home, CHSP or disability services.
• Strong administration and coordination skills.
• Excellent attention to detail.
• Confidence using multiple systems.
• Good understanding of plans and budgets.

Responsibilities

• Support Care Partners with administration and client plan coordination.
• Manage services against approved plans and budgets.
• Coordinate services and providers.
• Maintain accurate client records and documentation.
• Monitor plan utilisation and identify issues.
• Follow up actions to ensure services are delivered as planned.
• Liaise with internal teams, clients and external providers.
• Take ownership of administrative detail that keeps client services running smoothly.
