Qualifications

• Experience within Support at Home, Home Care Packages, NDIS or disability services.

Responsibilities

• Supporting Care Partners with the administration and coordination of client plans.
• Managing services against approved plans and budgets.
• Coordinating services and providers; Maintaining accurate client records and documentation.
• Monitoring plan utilisation and identifying issues.
• Following up actions and ensuring services are delivered as planned.
• Liaising with internal teams, clients and external providers.
• Taking ownership of the administrative detail that keeps client services running smoothly.
