Job summary

Alpine Health is seeking a part-time Care Partner in the Alpine region of Northeast Victoria. The role focuses on care management and service coordination under the Support at Home (SAH) program, aligning with aged care assessments and individual goals.

Collaboration with informal carers and family is encouraged with participant consent. Successful applicants will provide care planning, maintain monthly touchpoints, and participate in routine case conferencing and reviews while adhering to Aged

Responsibilities

• Care Management and Service Coordination
• Deliver person-centred care planning that reflects each participant’s individual needs, goals, and preferences and budget for clients presenting with Low to Medium Care needs (HCP level 1-3, SAH levels 1-5)
• Assist with Support at Home waitlist enquiries to facilitate clients to proceed to an admission within their 56 days allocated - where possible.
• Assist participants in understanding their approved services and how these can be delivered within their funding.
• Ensure care plans are consistent with reablement, independence, and wellbeing principles.
• Support participants in informed decision-making, including dignity of risk.
• Maintain regular communication with participants, providing a minimum monthly care management touchpoint.
• Conduct home visits for the purpose of admission and care planning on an occasional basis.
• Action Support Plan Reviews within MAC Portal for the purpose of client reassessment in consultation with Clinical Care Partner or Manager Support at Home.
• Actively participate in fortnightly Case Conferencing with Support at Home Manager and team
• Actively engage in regular supervision with Support at Home Manager.

$39.31 - $43.98 per hour
