Job summary

LiveHire Limited in Victoria is seeking a Care Partner for the Support at Home (SAH) program on an ongoing part-time basis (3 days per week). You will plan, manage and coordinate services aligned with aged care assessments and participant goals, collaborating with informal carers and family where appropriate.

The role requires experience in health and community settings and a Certificate III in health, nursing or a related field. Competitive remuneration and employee support are provided.

Qualifications

• Experience in health and community settings.
• Certificate III level qualification or higher in health, nursing, or community work.

Responsibilities

• Care management and service coordination for participants.
• Deliver person-centred care planning aligned with individual goals and budgets.
• Support with SAH waitlist enquiries to facilitate admissions within 56 days.
• Assist participants in understanding approved services and delivery within funding.
• Ensure care plans reflect reablement, independence and wellbeing.
• Maintain regular monthly care management touchpoints with participants.
• Conduct home visits for admission and care planning as needed.
• Contribute to case conferencing and supervision with the SAH team.
