Job summary

Bolton Clarke Group is seeking a Care Partner in Melbourne to act as a care manager under the Support at Home program. The role focuses on planning, coordinating and overseeing home-based care delivery with clinical team input when needed, working from our Forest Hill office.

You will collaborate with participants to develop individualised care plans, liaise with GPs and allied health providers, monitor service delivery and ensure documentation aligns with aged care quality standards.

Qualifications

• Experience in care coordination or case management in aged care, health or disability services.
• Strong understanding of Support at Home Program and Quality Standards.
• Excellent communication, empathy and cultural competence.

Responsibilities

• Develop individualized care plans with participants, incorporating goals, preferences and risk factors.
• Collaborate with GPs, specialists and allied health providers for integrated care.
• Review care notes and service delivery data to identify changes in participant needs.
• Support care management claims to comply with program rules.
• Assist participants and carers to make informed service decisions.
