Job summary

Bolton Clarke is seeking a dedicated Care Partner to manage care plans and coordinate services for older adults living at home in Melbourne, based in Forest Hill. You will work with clinical teams to align care with individual goals and rights, ensuring safe, respectful, and culturally appropriate support.

As part of the Home & Community Support team, you will collaborate with participants, carers and healthcare professionals to deliver comprehensive care while monitoring resources and ensuring

Qualifications

• Be responsible for delivering care management activities enabling older people to live safely and independently at home.
• Collaborate with participants to develop individualised care plans incorporating goals, preferences, culture, risk factors and AT-HM requirements.
• Collaborate with GPs, specialists, and allied health providers to ensure integrated care.
• Maintain accurate care notes and documentation supporting outcomes under standards.

Bolton Clarke Residential group now has 87 Aged Care Homes across QLD, NSW, VIC, SA & WA, with the inclusion of the Allity, McKenzie & Acacia Living Group. **About the role**The Care Partner is the designated care manager under the Support at Home Program, responsible for delivering care management activities that enable older people to live safely and independently at home. You will play a key role in planning and overseeing care service delivery across all ongoing support classifications within scope and work closely with the appropriate clinical team members when a participant’s needs require clinical input or complex care planning. The role will be based in our Forest Hill office.**As a Care Partner, you will:*** Collaborate with participants to develop individualised care plans.
