Catholic Healthcare Limited is seeking a Care Partner for a hybrid role based in New Lambton, NSW, visiting clients across the Hunter region and working from home. The permanent full-time position offers $48.00 per hour plus superannuation and Km reimbursement, with up to $18,550 tax-free via salary packaging.

As a Care Partner, you will deliver relationship-centred care, manage client portfolios, and work with multi-disciplinary teams to achieve exceptional outcomes and wellbeing for clients

Qualifications

• Thorough understanding of Aged Care Legislation and Aged Care Quality Standards.
• Strong understanding of ageing and related health issues.
• Case management including assessment, care planning, goal writing and reviewing.
• Experience in a community setting in a similar role.
• Ability to work independently and in a multi-disciplinary team.
• Excellent digital literacy including Outlook and Excel.

Responsabilités

• Achieve billable hours and service delivery expectations through care planning, funding utilisation, billing accuracy, and proactive client portfolio management.
• Undertake person-centred care planning in line with Catholic Healthcare’s model of care.
• Educate clients and families on program guidelines, inclusions/exclusions, and potential contributions.
• Coordinate with clinical, allied health and operational teams to identify care needs and plan interventions.
• Use screening, assessment and risk dashboards to identify high-risk clients and prioritise reviews.
• Initiate case conferences to support hospital, rehab or respite discharge planning.
• Promote Catholic Healthcare value proposition and enable clients to live at home with choice and safety.
• Ensure accurate billing, timely documentation and alignment with funding availability.
• Adhere to Aged Care Standards in all interactions and documentation.
