Catholic Healthcare Limited is seeking a Care Partner to deliver relationship-centred care management for clients living at home in Northern Sydney and surrounding areas. The role is office-based with visits to clients, Monday to Friday, permanent full-time.

You will manage care plans, coordinate with clinical teams, and help clients access services while aligning with Aged Care standards and funding where applicable.

Competenze

• Solid understanding of Aged Care Legislation and Quality Standards.
• Experience in case management including assessments, planning and reviews.
• Experience in community-based settings.
• Able to work independently and in multi-disciplinary teams.
• Proficient with common digital tools (Outlook, Excel).

• Develop and monitor care plans, plan interventions and review outcomes.
• Educate clients and families on program guidelines and cost sharing.
• Coordinate with clinical, allied health and operational teams to meet care needs.

• $48.00 per hour + superannuation + Km reimbursement ( $1.11 per km)

In the challenging and rewarding role of Care Partner, you will be providing relationship-centred care management that delivers exceptional client outcomes, strengthens wellbeing, and supports clients to thrive and live safely at home.

Day To Day This Role Looks Like

• Achieving billable hours and service delivering expectations through effective care planning, funding utilisation, billing accuracy, and proactive management of client portfolio
• Undertaking person-centredcare planningin accordance withCatholic Healthcare's model of care
• Educating clients and their families on program guidelines, inclusions and exclusions and possible financial contributions
• Communicating and coordinating with members of the clinical, allied healthand operational teamstoidentifyclientcare needs, planinterventions, and review outcomes of care
• Using screening,assessmentand risk dashboards toidentifyhigh riskclients and prioritizetimelyreviews
• Initiating case conferencing to support hospital, rehab or respite discharge planning forwellbeing interventions, serviceadjustmentsandtimelyreferrals
• Communicating and coordinating with members of the clinical, allied healthand operational teamstoidentifyclientcare needs, planinterventions, and review outcomes of care
• Promoting Catholic Healthcare Value proposition and service offerings while enabling clients to live at home independently with choice, control and connecting them to services that are safe,personaland effective
• Ensuringaccuratebilling,timelydocumentationand alignment of services with client goals and funding availability
• Understanding and adhering to the Aged Care Standards in all interactions, practices and documentation

To Be Successful In This Role You'll Need

• Thorough understanding of the Aged Care Legislation and Aged Care Quality Standards
• Strong understanding of the ageing process, particularly age-related health issues
• Case management experience including assessment, care planning, goal writing and reviewing
• Previousexperience working within acommunity settingin a similar role
• Ability to work independently and as part of a multi-disciplinary team
• Excellent digital literacy, including Microsoft Outlook and Excel
