Catholic Healthcare Limited is seeking a dedicated Care Partner to provide relationship-centred care at home across the Central Coast from Wyong to Umina. The role is permanent full-time, Monday to Friday, with office base and client visits.

Earn $48.00 per hour plus superannuation and km reimbursement, with tax-advantaged salary packaging. Key duties include effective care planning, funding utilization, billing accuracy, and collaborative case management with a multidisciplinary team.

Qualifications

• Thorough understanding of Aged Care Legislation and Quality Standards.
• Excellent digital literacy, including Microsoft Outlook and Excel.
• Ability to work independently and as part of a multi-disciplinary team.

Responsibilities

• Deliver relationship-centred care management to achieve exceptional client outcomes.
• Plan care, manage funding usage, ensure accurate billing and timely documentation.
• Coordinate with clinical and allied health teams to identify needs and plan interventions.
• Promote value propositions and enable clients to live at home with independence.
