Catholic Healthcare Limited is seeking a Care Partner for an office-based role in Hunters Hill with travel across the Northern Sydney area. The position is permanent full-time, Monday to Friday, offering $48.00 per hour plus superannuation and kilometre reimbursement ($1.11 per km).

Responsibilities include relationship-centred care management, care planning, budgeting and billing accuracy to support clients living safely at home. Strong collaboration with a multidisciplinary team is essential.

Competenze

• Demonstrated experience in case management within aged care, community care or government-funded health settings.
• Strong understanding of government funding models, including budgeting and compliance requirements.
• Experience managing care plans, assessments and client outcomes.

Mansioni

• Achieving billable hours and service delivery through care planning and accurate billing.
• Undertaking person-centred care planning according to Catholic Healthcare’s model of care.
• Educating clients and families on program guidelines and possible contributions.
• Coordinating with clinical, allied health and operational teams to identify care needs.
• Using risk dashboards to identify high-risk clients and timely reviews.
• Initiating case conferencing to support discharge planning and wellbeing interventions.
• Promoting Catholic Healthcare values and assisting clients to live at home with choice and independence.
• Ensuring accurate billing, timely documentation and alignment with funding.
