Australian Vietnamese Women’s Association (AVWA) in Melbourne seeks a Care Partner to deliver care management under the Support at Home Program. You will coordinate person-centered plans to enable older people to live safely at home, with a focus on wellness, reablement and culturally safe care.

You will collaborate with clinicians, GPs and carers, develop budgets, monitor risks, and ensure timely care planning and documentation.

Qualifikationen

• Diploma in Community Services, Aged Care, Case Management, or equivalent experience.
• Experience in care coordination or case management in aged care, health, or disability services.
• Solid understanding of the Support at Home Program, Quality Standards, and aged care rights.
• Ability to interpret clinical guidance and escalate appropriately.
• Strong communication, empathy and cultural competence.
• Proficient with care management or CRM systems.
• Police check or NDIS Worker Screening Check, and full COVID-19 vaccination.
• First Aid/CPR certification.
• Proficient in English and Vietnamese communication.

Aufgaben

• Develop individualized care plans within 28 days of service commencement.
• Coordinate care across AT-HM and other streams with clinicians.
• Review care notes and monitor risks; conduct monthly activities and annual plan reviews.
• Manage budgets and assist with service negotiations under program rules.
• Communicate with GPs, allied health, and service coordinators for coordinated care.
• Provide brokerage services including initial home safety assessments.
• Be flexible with hours and participate in staff meetings.
