Job summary

Just Better Care Melbourne North and North West is hiring a Care Partner to manage a caseload of around 70 Support at Home participants, ensuring the right plan, team and funding are in place.

You will assess needs, build care plans with families and health professionals, coordinate rostering with the team, monitor plans, manage participant budgets and build strong relationships with GPs and allied providers.

Qualifications

  • Experience in case management or care coordination in aged or community care.
  • Clear, warm communication with participants, families and clinicians.
  • Current driver's licence and police check or NDIS Worker Screening Check.
  • A Certificate III/IV in ageing, disability or community services, or a nursing background, is well regarded.

About us

Just Better Care Melbourne North and North West supports older people to live well at home. We are a locally run team, office based in Preston. Supporting clients in the North and North West suburbs.

About the role

As a Care Partner, you'll own a caseload of around 70 Support at Home participants and report to the General Manager. You are the person who makes sure each participant's care works: the right plan, the right team, and funding put to great use.

You will:

Assess needs and build care plans with participants, their families and health professionals

Work with the rostering team to build the care team, including support workers and contractors

Stay in contact with every participant, reviewing plans as needs change

Build strong working relationships with client's support network, e.g., GPs, allied health and third-party providers

About you

Genuine care for older people, shown in how you listen, follow through and advocate for them

Commercial acumen: you understand that well-used funding and a sustainable service go together, and you notice referral and growth opportunities

Agility: Support at Home is still bedding in, so you adapt quickly when rules, systems or a participant's needs change

A real appetite to learn, whether that's new funding rules, new systems or feedback on your own practice

Experience in case management or care coordination in aged or community care

Working knowledge of Support at Home (or Home Care Packages) guidelines and funding

Clear, warm communication with participants, families and clinicians

A current driver's licence and police check or NDIS Worker Screening Check

A Certificate III/IV in ageing, disability or community services, or a nursing background, is well regarded.

What we offer

$85,000–$95,000 + superannuation, depending on experience
