Job summary

Anglicare Sydney is seeking a Care Partner - Villages to support older Australians in accessing care, services and government funding within Warriewood Brook Village. This permanent, full-time, onsite role runs Monday to Friday 8:30am–4:30pm with minimal travel within the village.

You will guide residents and families through their care journey, develop tailored care plans, coordinate funding, and collaborate with village teams and healthcare professionals to ensure high-quality outcomes.

Qualifications

• Diploma in Community Services, Service Coordination or a related discipline.
• Experience within aged care, community services, home care or case management environments.
• Experience conducting comprehensive assessments and developing care plans.
• Current Driver's License and access to a vehicle with minimum third-party insurance.

Responsibilities

• Conduct comprehensive assessments of residents' needs, goals and preferences.
• Develop individualised care plans and budgets that support resident wellbeing and independence.
• Coordinate My Aged Care assessments and support residents to access appropriate funding and services.
• Ensure residents are maximising available funding and receiving the right level of care and support.
• Monitor changing care needs and advocate for residents to access additional services where required.
• Coordinate referrals to healthcare professionals, service providers and community supports.
• Deliver an exceptional customer experience to prospective and existing residents.
• Maintain accurate care plans, case notes and resident documentation.
• Monitor service delivery and ensure support services align with resident goals and budgets.
• Build strong relationships with residents, families, healthcare professionals and service providers.
• Collaborate with Village Teams and internal stakeholders to achieve positive resident outcomes.
• Promote a safe, clean and respectful workplace.

What You'll Bring

You are a compassionate and customer-focused professional who enjoys helping older people navigate the aged care system and access services that improve their quality of life.

What's Required

• Diploma in Community Services, Service Coordination or a related discipline

• Previous experience within aged care, community services, home care or case management environments

• Experience conducting comprehensive assessments and developing care plans

• Current Driver's License and access to a vehicle with minimum third-party insurance
