Job summary

Anglicare Sydney is seeking a Care Manager based at the Rooty Hill Residential Care home to lead clinical care delivery and champion resident well-being.

You will work with RN colleagues, Allied Health, and external providers to ensure safe, person-centred care; you will mentor staff and drive quality and governance.

This permanent full-time role offers opportunities to influence outcomes and contribute to compassionate care in a respected not-for-profit.

Qualifications

• Current AHPRA Registration as a Registered Nurse (Division 1).
• Previous leadership experience within aged care or residential care.
• 5 years of strong clinical assessment, care planning and clinical governance knowledge.
• Experience leading multidisciplinary teams.

Responsibilities

• Oversee the delivery of high-quality clinical care and achievement of positive resident outcomes.
• Ensure care plans are developed, reviewed and updated to reflect resident needs.
• Monitor resident health, wellbeing and clinical outcomes through effective care management.
• Support multidisciplinary care planning involving residents, families and healthcare professionals.
• Ensure clinical benchmarks, infection control and quality standards are maintained.
• Lead, mentor and support Registered Nurses and Allied Health team members.
• Foster a culture of accountability, teamwork, learning and continuous improvement.
• Conduct performance reviews and support staff development and capability building.
• Manage performance, conduct and workforce issues in partnership with the Residential Manager.
• Build positive relationships with residents, families and representatives.
• Ensure families are informed and supported throughout their loved one's care journey.
• Ensure care delivery aligns with organisational policies, professional standards and regulatory requirements.
• Support documentation quality, funding compliance and clinical reporting requirements.
• Support AN-ACC care minutes achievement and clinical funding requirements.
• Contribute to clinical reviews, audits and quality improvement initiatives.
• Support best practice clinical outcomes through education and continuous improvement.
• Promote a safe, respectful and inclusive environment for residents and staff.
• Support workplace health and safety requirements and reporting obligations.

This is a permanent full-time opportunity based at our Rooty Hill Residential Care home.

As a Care Manager, you will be responsible for the leadership and oversight of clinical care delivery within our Residential Care community. Working closely with the Residential Manager, Registered Nurses, Allied Health professionals and external healthcare providers, you will ensure residents receive safe, effective and person-centred care aligned with best practice standards.

You will provide clinical leadership, support care planning, monitor resident outcomes and foster a high-performing team culture that delivers exceptional resident experiences.

What’s Required
