Job summary

Australian Unity is seeking a Care Partner to support customers in their care journey, helping them stay independent and live safely at home or in a preferred setting.

The role is permanent full-time with flexible hours and a hybrid work model (clinic visits, branch days, and WFM). You will coordinate holistic care plans, collaborate with clinical teams, families and external providers to deliver person-centred care and optimise outcomes.

Qualifications

• Experience in onboarding assessments and developing holistic care plans.
• Coordination of multidisciplinary services and care transitions.
• Ability to build trusted relationships with customers and families.
• Knowledge of funding streams, quality standards, and compliance.

As a Care Partner, you'll support customers throughout their care journey, helping them achieve their goals and remain living safely and independently at home or in a place of their choosing. Working closely with customers, families, clinical teams and external providers, you'll develop holistic care plans, coordinate services and ensure customers receive person-centred support.

About the role:

This is a Permanent Full-Time opportunity offering flexibility and a great work-life balance.

• Flexible work hours: 8:30am - 4:30pm or 9:00am - 5:00pm, Monday to Friday
• Attractive remuneration of $45.56 per hour plus superannuation
• Hybrid role combining Client visits, Branch Office Days (Maclean- 3 days) & WFM (2 days)
• Supporting SAH customers.

Key Accountabilities:

• Build trusted relationships with customers and support them to achieve their goals at home.
• Conduct onboarding assessments, develop holistic care plans, coordinate multidisciplinary services and complete customer reviews as needs change.
• Collaborate with clinical teams, families and external providers to deliver safe, person-centred care and achieve optimal customer outcomes.
• Support customers through care transitions, including hospital discharge, changes in health needs and offboarding activities where required.
• Identify unmet needs, optimise funding utilisation and support access to services that enhance customer outcomes.
• Recognise and elevate customer risks, safeguarding concerns and changing care needs through appropriate pathways.
• Maintain accurate customer records in enterprise systems and contribute to quality, compliance and continuous improvement activities.
• Promote safety, quality and person-centred care across all aspects of service delivery.

What You'll Bring:

• Experience in community, aged care, disability or health services, including case management and coordinating multidisciplinary care.
• Strong customer focus, with the ability to build trusted relationships and navigate sensitive conversations with empathy and professionalism.
• Excellent communication skills and confidence using Microsoft Office and customer management systems.
• Knowledge of funding streams, quality standards, audit requirements and care transitions, including discharge planning.
• Strong decision-making, problem-solving, time management, and prioritisation skills in a fast-paced environment.
• A collaborative mindset, with the ability to work effectively across multidisciplinary teams, families, clinicians and external providers.
• Relevant qualifications in Community Services, Aged Care or a related discipline (Certificate IV or Diploma desirable).
