Job summary

Australian Unity is seeking a Care Partner to guide customers through their care journey with compassion, coordinating services with families, clinical teams and external providers to deliver person-centred care.

This permanent full-time role offers flexible hours, a hybrid arrangement with 2 days working from home and 3 days in the Birtinya office, and an attractive remuneration of $45.56 per hour plus superannuation.

Qualifications

• Experience in community or aged care.
• Knowledge of SAH is advantageous.
• Strong customer focus with empathy.
• Excellent verbal and written communication skills.
• Knowledge of funding streams, quality standards and audits.
• Experience supporting care transitions such as discharge planning.
• Strong decision-making and time management.
• Collaborative mindset in a team-based environment.
• Relevant qualifications (Certificate IV or Diploma in Community or Aged Care desirable).

Responsibilities

• Build trusted relationships with customers and support them to achieve their goals at home.
• Conduct onboarding assessments, develop holistic health plans, and coordinate multidisciplinary services.
• Collaborate with clinical teams, families, and external providers to deliver safe, person-centred care and optimal outcomes.
• Support customers through care transitions, including hospital discharge or changes in health needs, and follow up after offboarding when needed.
• Maintain accurate records in enterprise systems, participate in audits, and contribute to quality and continuous improvement initiatives.
• Identify opportunities for additional services to enhance customer outcomes and contribute to service growth.
• Promote safety, quality, compliance and a culture of continuous improvement across all activities.

Working closely with families, clinical teams, and external providers, you’ll develop holistic health plans, coordinate services, and ensure customers receive safe, person-centred care that enables them to age well at home or in a place of their choosing.

About the role:

This is aPermanent Full-Timeopportunity offering flexibility and a great work-life balance.

• Flexible work hours: 8:30am – 4:30pm or 9:00am – 5:00pm, Monday to Friday
• Attractive remuneration of$45.56 per hour plus superannuation
• Hybrid work model with 2 days working from home and 3 days in the Birtinya office
• Supporting SAH customers across the Sunshine Coast community

Key Accountabilities

What You’ll Bring

• Experience in community or aged care and knowledge of SAH would be advantageous but not essential
• Strong customer focus, with the ability to build trusted relationships and handle sensitive conversations with empathy
• Excellent verbal and written communication skills & tech savvy with Microsoft Suite and CRM systems
• Knowledge of funding streams, quality standards, audits, and experience supporting care transitions such as discharge planning
• Strong decision-making, problem-solving, time management, and adaptability in a fast-paced environment
• Collaborative mindset, able to work effectively in a team-based, matrix environment, liaising with families, clinicians, and external providers
• Relevant qualifications (Certificate IV or Diploma in Community or Aged Care desirable)
