Job summary

Australian Unity is seeking a dedicated Care Partner to guide customers through their care journey, coordinating holistic health plans with clinical teams, families, and external providers. The role supports age-at-home goals and person-centred care across Southern Highlands, with some days from home in a hybrid setup.

Ideal candidates have community or aged care experience, strong communication, and familiarity with funding streams and audits.

Qualifications

• Experience in community or aged care, including case management and coordinating multidisciplinary care in the home.
• Strong customer focus with empathy.
• Excellent verbal and written communication and tech savvy with Microsoft Suite and CRM systems.
• Knowledge of funding streams, quality standards, audits, and care transitions such as discharge planning.
• Strong decision-making, problem-solving, time management and adaptability.

Responsibilities

• Build trusted relationships with customers and support them to achieve their goals at home.
• Conduct onboarding assessments, develop holistic health plans, and coordinate multidisciplinary services.
• Collaborate with clinical teams, families, and external providers to deliver safe, person-centred care.
• Support customers through care transitions, including hospital discharge or changes in health needs, and follow up on offboarding where needed.
• Maintain accurate records in enterprise systems, participate in audits, and contribute to quality and continuous improvement initiatives.
• Identify opportunities for additional services and support to enhance customer outcomes and contribute to service growth.
• Promote safety, quality, compliance, and a culture of continuous improvement across all activities.

As aCare Partner, you’ll guide customers through their care journey with compassion. Working closely with families, clinical teams, and external providers, you’ll develop holistic health plans, coordinate services, and ensure customers receive safe, person-centred care that enables them to age well at home or in a place of their choosing.

This is a full time, hybrid role involving customer visits across the Southern Highlands areas of Bowral, Moss Vale etc, working from Campbelltown office and 2 days from home.

Key Accountabilities

What You’ll Bring

• Experience in community or aged care, including case management and coordinating multidisciplinary care in the home.
• Strong customer focus, with the ability to build trusted relationships and handle sensitive conversations with empathy.
• Excellent verbal and written communication skills & tech savvy with Microsoft Suite and CRM systems
• Knowledge of funding streams, quality standards, audits, and experience supporting care transitions such as discharge planning.
• Strong decision-making, problem-solving, time management, and adaptability in a fast-paced environment.
• Collaborative mindset, able to work effectively in a team-based, matrix environment, liaising with families, clinicians, and external providers.
• Relevant qualifications (Certificate IV or Diploma in Community or Aged Care desirable).
