Australian Unity is hiring a Care Partner in Queensland. The role focuses on guiding customers through their care journey with compassion, partnering with families, clinical teams and external providers to build holistic health plans and coordinate services.

This permanent full-time role offers flexible hours, a hybrid work model, and remuneration of $45.56 per hour plus superannuation. You will support SAH customers across the Sunshine Coast community.

Qualifications

• Experience in community or aged care is preferred.
• Strong customer focus with empathetic communication.
• Excellent verbal and written communication; tech savvy.
• Knowledge of funding streams and audits helpful.
• Ability to manage time and adapt in a fast-paced environment.
• Certificate IV or Diploma in Community or Aged Care desirable.

• Attractive remuneration of $45.56 per hour plus superannuation
• Hybrid work model with 2 days working from home and 3 days in the Birtinya office

Key Accountabilities

• Build trusted relationships with customers and support them to achieve their goals at home
• Conduct onboarding assessments, develop holistic health plans, and coordinate multidisciplinary services
• Collaborate with clinical teams, families, and external providers to deliver safe, person‑centred care and optimal outcomes
• Support customers through care transitions, including hospital discharge or changes in health needs, and follow up with offboarding where needed
• Maintain accurate records in enterprise systems, participate in audits, and contribute to quality and continuous improvement initiatives
• Identify opportunities for additional services and support to enhance customer outcomes and contribute to service growth
• Promote safety, quality, compliance, and a culture of continuous improvement across all activities

What You'll Bring

• Experience in community or aged care and knowledge of SAH would be advantageous but not essential
• Strong customer focus, with the ability to build trusted relationships and handle sensitive conversations with empathy
• Excellent verbal and written communication skills & tech savvy with Microsoft Suite and CRM systems
• Knowledge of funding streams, quality standards, audits, and experience supporting care transitions such as discharge planning
• Strong decision‑making, problem‑solving, time management, and adaptability in a fast‑paced environment
• Collaborative mindset, able to work effectively in a team‑based, matrix environment, liaising with families, clinicians, and external providers
• Relevant qualifications (Certificate IV or Diploma in Community or Aged Care desirable)
