Job summary

Rumbalara Aboriginal Co-operative invites applications for two Care Manager (Case Management) roles based in Shepparton or Swan Hill, 38 hours per week, on a fixed-term 24-month contract. These community-based positions support Elders under the Support at Home program, focusing on culturally safe, person-centred care.

The roles require qualifications in aged care, community services, social work, or related fields, with experience helping older people live independently.

Qualifications

• Qualification in Aged Care, Community Services, Social Work, Case Management, or related discipline.
• Experience supporting older people to live independently in the community.
• Knowledge of aged care programs (Support at Home, Home Care Packages, CHSP) and brokerage models.
• Understanding of person-centred care and consumer choice principles.
• Awareness of the health, social, and cultural needs of Aboriginal and Torres Strait Islander Elders and communities.
• Ability to assess client needs and develop coordinated support plans.
• Experience using client management systems and maintaining accurate records.
• Strong communication, organisational, and interpersonal skills.
• Ability to work independently and within a multidisciplinary team.

Responsibilities

• Coordinate and manage services for participants under the Support at Home Program.
• Identify goals, support needs, and preferences with clients.
• Ensure services are culturally safe, person-centred and aligned with guidelines and standards.
• Support clients to live safely and independently at home.
• Collaborate with multidisciplinary teams and partner organisations.
