Job summary

Bupa Ballarat is seeking a Care Manager to join our modern Delacombe care home in Ballarat. You will support the General Manager in delivering person-centred care, ensuring continuous assessment, care planning, and robust clinical governance to help residents live healthier, longer lives.

You will oversee a dedicated clinical team, manage medications, lead AN-ACC compliance, and collaborate with health practitioners to optimise resident outcomes.

Qualifications

• Bachelor of Nursing with current AHPRA registration.
• Experience managing a minimum of 30 beds.
• Proven track record in delivering quality clinical care in aged care or acute settings.
• Excellent clinical assessment and care planning capabilities.
• Strong communication, leadership and relationship management skills.

Responsibilities

• Oversee daily clinical operations of the care home.
• Lead and develop the care home clinical team, including coaching and education.
• Review clinical incidents, complaints, and changes.
• Lead compliance with AN-ACC assessments.
• Manage person-centred care planning and evaluation for residents.
• Prepare and conduct clinical governance audits.
• Oversee medication management in line with resident needs and regulations.
• Collaborate with allied health and medical practitioners to optimise resident care.

The heart of the role

As Care Manager you will work as part of a team of empathetic and caring clinical professionals. You will support the General Manager in managing and leading the delivery of person-centred care for residents, ensuring the highest standard of ongoing assessment, care planning and evaluation, and clinical governance—helping them to live longer, healthier, and happier lives.

Duties include

• Oversee the daily clinical operations of the care home
• Support and lead the care home clinical team including coaching, mentoring, staff allocations and delegation, performance management and education
• Review clinical incidents, complaints and changes
• Lead compliance with AN-ACC assessments
• Manage person-centred care planning and evaluation for residents, including case conferencing with families and carers
• Prepare and conduct clinical governance audits
• Oversee medication management in accordance with resident needs and legislative requirements
• Liaise with allied health, medical practitioners and other services to provide optimal resident care

What will you bring?

• Bachelor of Nursing with current AHPRA registration (essential)

• Excellent clinical assessment skills and care planning capabilities
• Highly effective communication, leadership, and relationship management skills
• Understanding of Resident Rights, Aged Care Accreditation Standards and Outcomes
