Job summary

Bupa Bellarine Lakes in Bellarine Lakes, Victoria, is recruiting a Care Manager to lead the delivery of person-centred care and to support the General Manager in clinical governance.

You will oversee daily operations, coach staff, conduct care planning and AN-ACC readiness, and foster a compassionate team environment to enhance residents' well-being.

Qualifications

• Bachelor of Nursing with current APHRA is essential.
• Post grad qualification in aged care highly regarded.
• Strong and effective leadership, communication and interpersonal skills.
• A person-centred care mindset.
• Passion for nurturing a strong team environment.
• Empathy for residents.
• Proven track record in delivering quality clinical care in aged care or acute settings.
• Excellent clinical assessment and triaging skills and care planning.
• Experience in fostering a reporting culture with investigative skills.
• Understanding of Resident Rights and Aged Care Standards.

Responsibilities

• Oversee the daily clinical operations of the care home ensuring residents receive the best care available.
• Support and lead the care home clinical team including coaching, mentoring, allocations, delegation and education.
• Review clinical incidents and complaints and provide solutions to reduce risks.
• Lead compliance with AN-ACC assessments.
• Manage person-centred care planning and evaluation for residents with families.
• Foster a strong team environment.
• Prepare and conduct clinical governance audits.

The heart of the role

As a Care Manager, you’ll lead with purpose - supporting your team and making a meaningful difference to the lives of our residents every day. You will support the General Manager in managing and leading the delivery of person-centred care of residents, ensuring the highest standard of ongoing assessment, care planning and evaluation and clinical governance.

What will you do?

• Oversee the daily clinical operations of the care home ensuring our residents receive the best care available.
• Support and lead the care home clinical team including coaching, mentoring, staff allocations and delegation, performance management and education.
• Review clinical incidents, complaints and changes, providing solutions to reduce any risks.
• Lead compliance with AN-ACC assessments.
• Manage person-centred care planning and evaluation for residents - including case conferencing with families and carers.
• Fostering a strong team environment.
• Prepare and conduct clinical governance audits.

• An understanding of Resident Rights, Aged Care Accreditation Standards and Outcomes.
