Job summary

Bupa Ballarat is seeking a Care Manager to lead the delivery of person-centred care for residents and support the General Manager. You will ensure ongoing assessment, care planning, evaluation and clinical governance to help residents live longer, healthier lives.

Your role includes leading the clinical team, managing daily operations, and ensuring compliance with standards, while maintaining strong engagement with families and external healthcare providers.

Qualifications

• Bachelor of Nursing with current AHPRA registration (essential).
• Experience managing a minimum of 30 beds.
• Proven track record delivering quality clinical care in aged care or acute settings.
• Excellent clinical assessment and care planning capabilities.
• Strong communication, leadership, and relationship management skills.
• Understanding of Resident Rights and Aged Care Accreditation Standards.

Responsibilities

• Oversee daily clinical operations of the care home.
• Lead and coach the care home clinical team with delegation and education.
• Review clinical incidents, complaints and changes.
• Lead compliance with AN-ACC assessments.
• Manage person-centred care planning and family conferencing.
• Prepare and conduct clinical governance audits.
• Oversee medication management per resident needs and laws.
• Liaise with allied health, medical practitioners and services.

The heart of the role

As Care Manager you will work as part of a team of empathetic and caring clinical professionals. You will support the General Manager in managing and leading the delivery of person-centred care for residents, ensuring the highest standard of ongoing assessment, care planning and evaluation, and clinical governance—helping them to live longer, healthier, and happier lives.

Duties include

• Oversee the daily clinical operations of the care home
• Support and lead the care home clinical team including coaching, mentoring, staff allocations and delegation, performance management and education
• Review clinical incidents, complaints and changes
• Lead compliance with AN-ACC assessments
• Manage person-centred care planning and evaluation for residents, including case conferencing with families and carers
• Prepare and conduct clinical governance audits
• Oversee medication management in accordance with resident needs and legislative requirements
• Liaise with allied health, medical practitioners and other services to provide optimal resident care

What will you bring?

• Bachelor of Nursing with current AHPRA registration (essential)
