Bupa Bellarine Lakes in Victoria seeks a Care Manager to lead the delivery of person-centred care, support the General Manager, and uphold clinical governance across the home. You will guide the clinical team to ensure high-quality care and compliance with standards.

You will oversee daily operations, empower the care team, manage care planning and governance, and cultivate a supportive, resident-focused culture that values dignity and excellence.

Qualifications

• Bachelor of Nursing with APHRA registration.
• Post-grad qualification in aged care is highly regarded.
• Strong leadership, communication and interpersonal skills.
• People-centred care mindset is essential.
• Proven track record delivering quality clinical care in aged care or acute settings.
• Excellent clinical assessment and triage skills and strong care planning capabilities.
• Understanding of Resident Rights and Aged Care Standards.

Responsibilities

• Oversee the daily clinical operations of the care home to ensure the highest standard of care.
• Support and lead the care home clinical team including coaching, mentoring, staff allocations and delegation.
• Review clinical incidents, complaints and changes, providing solutions to reduce risks.
• Lead compliance with AN-ACC assessments.
• Manage person-centred care planning and evaluation for residents, including family case conferencing.
• Foster a strong team environment.
• Prepare and conduct clinical governance audits.

You will support the General Manager in managing and leading the delivery of person-centred care of residents, ensuring the highest standard of ongoing assessment, care planning and evaluation and clinical governance.

What will you do?

• Proven and demonstrated experience in fostering a reporting culture balanced with your effective investigative skills.
• An understanding of Resident Rights, Aged Care Accreditation Standards and Outcomes.
