Bupa Ballina is seeking a Care Manager to lead the delivery of person-centred care for residents, supported by a compassionate clinical team. You will work closely with the General Manager to ensure high-quality assessments, care planning, and clinical governance.

Essential: Bachelor of Nursing with current AHPRA registration; postgraduate aged care qualification is highly regarded; proven experience in aged care or acute settings; strong clinical assessment, leadership, and communication

The heart of the role:

As our Care Manager, you will work as part of a team of empathetic and caring clinical professionals. You will support the General Manager in managing and leading the delivery of person-centred care of residents, ensuring the highest standard of ongoing assessment, care planning and evaluation and clinical governance, helping them to live longer, healthier, and happier lives.

Duties include:

• Oversee the daily clinical operations of the care home
• Support and lead the care home clinical team including coaching, mentoring, staff allocations and delegation, performance management and education
• Review clinical incidents, complaints, and changes
• Lead compliance with AN ACC assessments
• Manage person-centred care planning and evaluation for residents - including case conferencing with families and carers
• Prepare and conduct clinical governance audits
• Oversee medication management in accordance with resident needs and legislative requirements
• Liaise with allied health, medical practitioners, and other services to provide optimal resident care

What will you bring?

• Bachelor of Nursing with current AHPRA is essential
• Post grad qualification in aged care highly regarded
• Proven track record in delivering quality clinical care within an aged care or acute setting
