Bupa Ballarat is seeking a Care Manager to lead clinical operations, ensure person-centred care, and uphold governance and accreditation standards. You will collaborate with the General Manager to deliver high-quality resident care across a five-resident community setting.

Qualifications

• Bachelor of Nursing with AHPRA registration (essential).
• Experience managing a minimum of 30 beds.
• Proven track record delivering quality clinical care in aged care or acute settings.
• Excellent clinical assessment and care planning skills.
• Strong communication, leadership and relationship management abilities.
• Understanding Resident Rights and Aged Care Accreditation Standards.

The heart of the role

As Care Manager you will work as part of a team of empathetic and caring clinical professionals. You will support the General Manager in managing and leading the delivery of person-centred care for residents, ensuring the highest standard of ongoing assessment, care planning and evaluation, and clinical governance—helping them to live longer, healthier, and happier lives.

Duties include

• Oversee the daily clinical operations of the care home

• Support and lead the care home clinical team including coaching, mentoring, staff allocations and delegation, performance management and education

• Review clinical incidents, complaints and changes

• Lead compliance with AN-ACC assessments

• Manage person-centred care planning and evaluation for residents, including case conferencing with families and carers

• Prepare and conduct clinical governance audits

• Oversee medication management in accordance with resident needs and legislative requirements

• Liaise with allied health, medical practitioners and other services to provide optimal resident care

What will you bring?

• Bachelor of Nursing with current AHPRA registration (essential)

• Experience managing a minimum of 30 beds

• Proven track record in delivering quality clinical care within an aged care or acute setting

• Excellent clinical assessment skills and care planning capabilities

• Highly effective communication, leadership, and relationship management skills

• Understanding of Resident Rights, Aged Care Accreditation Standards and Outcomes
