Job summary

Bolton Clarke is seeking a Care Manager to lead clinical governance, staff management and resident care at Coastal Waters Aged Care near Jervis Bay. The role focuses on AN-ACC, audits and continuous improvement to ensure high quality, personalised care across the home.

Lead RNs and PCWs, drive staff development, and uphold aged care standards in a supportive, community-focused environment. This is a full-time, on-site opportunity in NSW.

Qualifications

• Current Nursing Registration with AHPRA.
• Experience within Aged Care Sector as a Senior Registered Nurse or Clinical Nurse Leader (desirable).
• Infection Prevention and Control Qualification (desirable).
• Demonstrated knowledge of aged care standards, accreditation and auditing processes, and AN-ACC.
• Commitment to delivering high quality care and staff development, and to continuous improvement of the home.
• Solid leadership and excellent communication/interpersonal skills.

Responsibilities

• Delivery of high quality and personalised care to all residents.
• Knowledge of aged care standards and accreditation processes; respond to audits with continuous improvement.
• Develop, monitor and implement nurse care plans; assess, plan and evaluate care plans.
• Perform comprehensive assessments and investigate issues/incidents.
• Ensure compliance with aged care standards and Bolton Clarke policies.
• Supervise, train and mentor RNs and PCWs.

The Role

As the Care Manager, your responsibilities will expand across clinical governance, staff management, change management, AN-ACC and resident care. Your duties will include, but are not limited to:

• Commitment to ensuring the delivery of high quality and personalised care to all resident
• Knowledge of the aged care sector, standards
• Sound understanding of processes needed to respond to clinical and quality audits using the principles of continuous improvement.
• Developing, monitoring and implementing nurse care plans + assessing, planning and evaluating existing care plans
• Performing comprehensive assessments and investigating issues and incidents
• Participating in and ensuring all compliance is adhered to in accordance with aged care standards and Bolton Clarke policies and procedures.
• Actively supervising, training, and mentoring all RNs and PCWs

About You

• The successful candidate must meet the following criteria:
• Current Nursing Registration with AHPRA
• Experience within Aged Care Sector as a Senior Registered Nurse or Clinical Nurse Leader (desirable)
• Infection Prevention and Control Qualification (desirable)
• Demonstrated working knowledge of current aged care standards, accreditation and auditing processes, and AN-ACC
• Personal and professional commitment to delivering high quality care, the development of yourself and your staff, and the overall continuous improvement of the home
• Solid leadership skills along with excellent communication and interpersonal skills
• Valid working rights in Australia (Passport, Birth Certificate etc)
• Complete a police check and or a NDIS Workers Check (or willingness to obtain) prior to commencement
