Job summary

Bolton Clarke Group is seeking a Care Manager in Greater Western Sydney to oversee clinical governance, staff management and change initiatives across resident care. You will drive AN-ACC compliance and lead multidisciplinary teams to deliver high-quality care in a caring home environment.

The role requires an active Nursing Registration with AHPRA and strong leadership and communication skills to mentor RNs and PCWs while ensuring continuous improvement and compliance with aged care standards.

Qualifications

• Current Nursing Registration with AHPRA.
• Experience in aged care as a Senior Registered Nurse or Clinical Nurse Leader desirable.
• Infection prevention and control qualification desirable.
• Knowledge of aged care standards, accreditation and AN-ACC processes.
• Commitment to delivering high quality care and continuous improvement.
• Strong leadership and communication skills.
• Right to work in Australia and willingness to obtain police/NDIS checks.

Responsibilities

• Clinical governance and staff management across the home.

• Develop, monitor and implement nurse care plans and assess/ evaluate existing plans.
• Perform comprehensive assessments and investigate issues/incidents.

The Role

As the Care Manager, your responsibilities will expand across clinical governance, staff management, change management, AN-ACC and resident care. Your duties will include, but are not limited to:

• Commitment to ensuring the delivery of high quality and personalised care to all resident
• Knowledge of the aged care sector, standards
• Sound understanding of processes needed to respond to clinical and quality audits using the principles of continuous improvement.
• Developing, monitoring and implementing nurse care plans + assessing, planning and evaluating existing care plans
• Performing comprehensive assessments and investigating issues and incidents
• Participating in and ensuring all compliance is adhered to in accordance with aged care standards and Bolton Clarke policies and procedures.
• Actively supervising, training, and mentoring all RNs and PCWs

About You

The successful candidate must meet the following criteria:

• Current Nursing Registration with AHPRA
• Experience within Aged Care Sector asa Senior Registered Nurse or Clinical Nurse Leader (desirable)
• Infection Prevention and Control Qualification (desirable)
• Demonstrated working knowledge of current aged care standards, accreditation and auditing processes, and AN-ACC
• Personal and professional commitment to delivering high quality care, the development of yourself and your staff, and the overall continuous improvement of the home
• Solid leadership skills along with excellent communication and interpersonal skills
• Valid working rights in Australia (Passport, Birth Certificate etc)
• Complete a police check and or a NDIS Workers Check (or willingness to obtain) prior to commencement
