Job summary

Anglicare Sydney is seeking a Care Manager to lead clinical care delivery across our Kingswood Residential Care home. This permanent full-time, onsite role works with the Residential Manager, RN and Allied Health teams to ensure safe, person-centred care and positive resident outcomes.

You will provide clinical leadership, support care planning, monitor outcomes and drive a high-performing team culture with compassion and professionalism, in line with our values and regulatory standards.

Qualifications

• Current AHPRA registration as a Registered Nurse (Div 1).
• 3–4 years of leadership experience in aged care or residential care.
• 5 years' clinical assessment, care planning and governance experience.
• Experience leading multidisciplinary teams.
• Bachelor of Nursing or equivalent qualification.

Responsibilities

• Provide clinical leadership, oversight of care delivery and governance.
• Lead, mentor and develop Registered Nurses and Allied Health staff.
• Monitor resident outcomes and support care planning and reviews.
• Collaborate with Residential Manager and external providers to ensure high-quality care.
• Ensure compliance with policies, standards and regulatory requirements.

This is a permanent full-time onsite opportunity based at our Kingswood Residential Care home between 9:00 am to 5:00 pm.

As a Care Manager, you will be responsible for the leadership and oversight of clinical care delivery within our Residential Care community. Working closely with the Residential Manager, Registered Nurses, Allied Health professionals and external healthcare providers, you will ensure residents receive safe, effective and person-centred care aligned with best practice standards.
You will provide clinical leadership, support care planning, monitor resident outcomes and foster a high-performing team culture that delivers exceptional resident experiences.
With compassion, professionalism and strong clinical leadership, you will:

Clinical Care & Resident Outcomes

• Oversee the delivery of high-quality clinical care and achievement of positive resident outcomes
• Ensure care plans are developed, reviewed and updated to reflect resident needs
• Monitor resident health, wellbeing and clinical outcomes through effective care management
• Support multidisciplinary care planning involving residents, families and healthcare professionals
• Ensure clinical benchmarks, infection control and quality standards are maintained

Leadership & Team Development

• Lead, mentor and support Registered Nurses and Allied Health team members
• Foster a culture of accountability, teamwork, learning and continuous improvement
• Conduct performance reviews and support staff development and capability building
• Manage performance, conduct and workforce issues in partnership with the Residential Manager

Resident & Family Engagement

• Build positive relationships with residents, families and representatives
• Ensure families are informed and supported throughout their loved one's care journey

Clinical Governance & Compliance

• Ensure care delivery aligns with organisational policies, professional standards and regulatory requirements
• Support documentation quality, funding compliance and clinical reporting requirements
• Support AN-ACC care minutes achievement and clinical funding requirements

Quality & Continuous Improvement

• Contribute to clinical reviews, audits and quality improvement initiatives
• Support best practice clinical outcomes through education and continuous improvement

Safety & Workplace Culture

• Promote a safe, respectful and inclusive environment for residents and staff
• Support workplace health and safety requirements and reporting obligations

What You'll Bring

You are an experienced and compassionate clinical leader who is passionate about delivering outstanding care and supporting older Australians to live well.

What's Required

• Current AHPRA Registration as a Registered Nurse (Division 1)
• 3-4 years of previous leadership experience within aged care or residential care as a Care Manager
• 5 years of strong clinical assessment, care planning and clinical governance knowledge as a registered Nurse
• Experience leading multidisciplinary teams

• Computer literacy and confidence using clinical systems and documentation platforms
