Job summary

Dovida is seeking a dedicated care coordinator to support our Circle of Care for older Australians in the Gosford region. You will liaise with clients and families, develop care plans, and coordinate with Caregivers to deliver person-centred support.

Qualifications

• Located in or near Gosford
• Experience managing Support at Home or Home Care Package clients
• Experience working with older people in a care or health setting
• Strong organisation, time management and communication skills
• A current Australian driver’s licence
• Valid Australian working rights and willingness to complete a National Police Check
• A tertiary qualification in nursing, social work, allied health or a related field (optional)
• Knowledge of local health and community services (optional)
• Familiarity with the Aged Care Act 2024 and the Strengthened Aged Care Quality Standards (optional)

Responsibilities

• Meet prospective clients and families to understand their goals, strengths and needs.
• Develop clear care plans, service agreements and budgets, and explain them in plain terms.
• Partner with our Client Services Team on Caregiver matching and help new clients settle in.
• Conduct regular reviews, elevate changing risks and coordinate hospital or residential care transitions.
• Monitor client budgets and keep documentation accurate and audit-ready.

What you’ll bring

You are organised, calm under pressure and comfortable working on your own or as part of a team. You put each client’s rights and choices first, and you build trust quickly with clients, families and health professionals.

To thrive in this role, you will have:

• Located in or near Gosford
