Job summary

Dovida Australia is seeking a caring professional to support older Australians at home. You will help create care plans, coordinate services and work with caregivers to meet each client’s goals with empathy and clear communication.

Located in or near Gosford, you will manage Home Care Package clients, ensure compliance, and maintain accurate records, while enjoying training opportunities and a supportive team culture.

Qualifications

• Located in or near Gosford and eligible to work in Australia.
• Experience managing Support at Home or Home Care Package clients.
• Experience working with older people in care or health settings.
• Strong organisation, time management and communication skills.
• A current Australian driver's licence and willingness to complete a National Police Check.
• Desirable: tertiary qualification in nursing, social work or allied health.

Responsibilities

• Meet prospective clients and families to understand goals, strengths and needs.
• Develop clear care plans, service agreements and budgets explained in plain terms.
• Partner with Client Services Team on caregiver matching and client settling.
• Conduct regular reviews, escalate risks and coordinate hospital or residential care transitions.
• Monitor client budgets and maintain accurate, audit-ready documentation.

With your person-led approach, you will:

What you’ll bring

You are organised, calm under pressure and comfortable working on your own or as part of a team. You put each client’s rights and choices first, and you build trust quickly with clients, families and health professionals.

To thrive in this role, you will have:

• Located in or near Gosford
• Experience managing Support at Home or Home Care Package clients.
• Experience working with older people in a care or health setting.
• Strong organisation, time management and communication skills.
• A current Australian driver’s licence.
• Valid Australian working rights and willingness to complete a National Police Check.

It’s not essential, but it would be wonderful if you also have:

• A tertiary qualification in nursing, social work, allied health or a related field.
• Knowledge of local health and community services.
• Familiarity with the Aged Care Act 2024 and the Strengthened Aged Care Quality Standards.
