Job summary

Dovida is seeking an experienced Care Manager in Toowong, supporting clients across Ipswich and Brisbane City. You will manage a caseload with strong SaH knowledge, coordinate services and deliver person-centred care to older Australians living at home.

With solid aged care experience and a practical understanding of Support at Home funding and budgets, you’ll build trusted relationships with clients and their families while turning goals into actionable care plans.

Qualifications

• Demonstrated experience as a Care Manager, Case Manager or similar aged care professional.
• Strong, practical experience with the Support at Home program, including funding, budgets and service coordination.
• Experience managing a client caseload and coordinating multiple services and stakeholders.
• Strong understanding of person-centred care and the ability to turn individual needs and goals into practical care plans.
• Excellent communication and relationship-building skills — you’re a genuine people person who can build trust with clients, families and care teams.
• The confidence to have sometimes difficult conversations while keeping the client’s best interests at the centre.
• Strong organisation and attention to detail across care plans, budgets, documentation and competing priorities.
• A practical, solutions-focused approach and the ability to work independently while contributing positively to a team.

• Manage client budgets and funding with a focus on sustainable, appropriate service delivery.
• Maintain accurate care plans, documentation and funding records.
