Based in Sydney’s Hurstville and Bankstown area, you will lead and coordinate client care with a passionate team, build strong relationships, and maintain quality standards.

You’ll play a pivotal role in shaping the client experience from guiding new families through their care journey to ensuring ongoing support meets evolving needs.

Based in Sydney’s Hurstville and Bankstown area, you’ll work alongside a passionate and collaborative team, leading and coordinating client care while ensuring the highest quality standards are met. Key Responsibilities

• Build genuine, trusting relationships with clients, families, and representatives, becoming their go-to support
• Conduct regular care reviews (in person or via phone) to ensure services continue to meet client needs and expectations
• Collaborate with caregivers, families, and health professionals to ensure care plans are personalised and effectiveMaintain strong community connections by engaging with local services, health providers, and referral networks

About you

• A tertiary qualification in a health-related field (e.g. social work, allied health) with at least 1 year of experience working with seniors and Home Care Packages
• Excellent organisational skills with the ability to juggle priorities and adapt to changing needs
• You thrive in a team environment and are willing to offer assistance when needed.
• Strong communication skills, both written and verbal, with proficiency in Microsoft Office
• A valid Australian Driver’s Licence and the flexibility to travel when required
