In this role, you’ll work closely with clients and their families to understand their needs, create tailored care plans, coordinate services and make sure care continues to meet their goals as circumstances change.

About You

• Experience in care management, case management or a similar client-focused role.
• A good understanding of aged care, Home Care Packages or Support at Home.
• Strong relationship-building skills and the ability to work well with clients, families and stakeholders.
• High attention to detail, with sound administrative and record‑keeping capability.
• A practical, team‑focused approach, with the confidence to work independently when needed.
