Bayside City Council is seeking a Care Finder & Community Assessment Officer to support older people and those with disability in navigating services across office and field settings. The role focuses on safety, wellbeing, and independent living while contributing to a welcoming customer service culture.

Qualifications

• Tertiary qualification in health, allied health or social science (Bachelor Level).
• Minimum two years' experience with older people who are vulnerable or homeless or at risk of homelessness and assisting people with a disability.
• Experience in mental health sector is an advantage.
• Knowledge of My Aged Care, Care Finder Program and related services.
