Bayside City Council is seeking a Care Finder & Community Assessment Officer to support older people and people with disability, including those experiencing homelessness, to access services and live safely and independently.

Based at the Sandringham council office with field work, the role offers flexible working patterns, generous leave provisions and wellbeing programs, with a strong commitment to diversity and inclusion.

Qualifications

• Bachelor-level health/aligned social science qualification.
• Minimum two years' experience with older people experiencing vulnerability or homelessness.
• Experience within the mental health sector is advantageous.
• Understanding of My Aged Care, Care Finder Program and related home support programs.
• Excellent written and verbal communication across diverse backgrounds.
• Empathetic, respectful and professional client engagement.
• Team-oriented with a strong customer service mindset.
• Willingness to undergo criminal history check and Working with Children check.
• Valid Victorian Driver’s Licence.

Employment Status: Parental Leave Cover (Fixed term until October 2027)
Days & Hours of Work: 4 days (days to be negotiated) 30.4 hours per week
Competitive Salary: Band 6 - $105,907.09pro rata + 12% superannuation
Location: Council office in Sandringham and in the field

About the Role:

The Care Finder and Community Assessment Officer primarily supports older people and people with disability, particularly those experiencing homelessness, housing insecurity, or vulnerability, to navigate the service system and access support services that promote safety, wellbeing, and independent living.

Key Selection Criteria:

To be successful in this role, you will have:

• A tertiary qualification in a health, allied health or social science discipline (Bachelor Level).
• Minimum two years' experience working with older people who are experiencing vulnerability and/or homeless, or at risk of homelessness as well as assisting people of all ages with a disability.
• Experience within the mental health sector would be well regarded.
• A comprehensive understanding ofMy Aged Care,the Care Finder Program, CommonwealthHome Support Program, Support at Home Program,Single Assessment services and Home and Community Care for Younger People Program (HACC PYP).
• Excellent written and verbal communication skills including an ability to communicate with people from diverse backgrounds.
• An ability to identify client needs and respond appropriately in an empathetic, objective, respectful and professional manner.
• The ability to work as part of a team, and a strong commitment to maintaining a high standard customer service culture.

Mandatory pre-employment requirements:

• Be willing to undergo and achieve a satisfactory criminal history check.
• Provide or obtain an employment-based Working with Children check.
• Valid Victorian Driver’s Licence.
