Job summary

IUIH is hiring a Care Coordinator for MATSICHS Social Health to coordinate supports for Indigenous clients facing complex social and mental health challenges. You will handle intake, screening, and coordinate services in collaboration with clinical staff.

Key responsibilities include managing referrals, developing care plans, and ensuring follow-up across Caboolture, Margate, and Strathpine. Aboriginal or Torres Strait Islander status is essential for this role.

Qualifications

• Essential Aboriginal or Torres Strait Islander status is a genuine occupational requirement.
• Criminal history check is current and valid.
• Valid C Class drivers licence (QLD).
• Working with Children’s Check (Qld Blue Card) or AHPRA registration.
• Certificate or Diploma in Community Services, Mental Health or similar.

Responsibilities

• Act as a central entry point for referrals to Social Health services.
• Conduct initial intake, screening, and structured non-clinical assessments.
• Escalate complex or high-risk presentations to Clinical Care Coordinators or senior clinicians.
• Support development of care plans and coordination of services.
• Manage a caseload of clients with follow-up and engagement.
• Work across Caboolture, Margate and/or Strathpine as needed.
