IUIH is seeking a Regional Care Coordinator CCSS to implement Care Coordination and Supplementary Services within the ITC program in Southeast Queensland. Work collaboratively with patients and general practices to facilitate chronic disease management and aged care eligibility assessment as needed.

The role emphasizes culturally safe practices, coordinating with ACCHS communities, and providing direct or relief care coordination as required.

Qualifications

  • Current Criminal History Check.
  • Current C Class Drivers Licence (QLD).
  • AHPRA Registration.
  • Current Disability Worker Screening Check (Qld Yellow Card).
  • Registered or Endorsed Enrolled Nurse.
  • Qualification in Nursing.
  • Ability to travel across SEQ.

Responsibilities

  • Liaise with Aboriginal and Torres Strait Islander communities to assess health needs and coordinate services.
  • Develop networks in regional Indigenous communities to promote Chronic Disease programs.
  • Improve case management of referrals to specialist services for chronic conditions.

The Regional and Specialist team includes Integrated Specialist Services, Mental Health and Wellbeing Services, Primary Healthcare Services, Birthing and Early Childhood Services (South), and Child and Youth Services. Our purpose is to connect, guide, and empower teams and communities through culturally safe, transparent, and responsive ways. **About the Role?**As Regional Care Coordinator CCSS, your role is to implement the Care Coordination and Supplementary Services elements of the Integrated Team Care (ITC) program, with mainstream General Practices. The Regional Care Coordinator will work collaboratively with patients and designated general practices to facilitate a person with a chronic condition to manage their health in a way that will result in an optimal health outcome. The Regional Care Coordinator will also provide direct/relief Care Coordinator services as required. Working collaboratively with patients and ACCHS’s to facilitate a person with a chronic condition to manage their health in a way that will result in an optimal health outcome and as required, facilitate Aged Care eligibility and needs assessment processes and contribute to the development of client care plans.
