Job summary

Mind Australia Limited is seeking a Care Coordinator in Warrnambool on a fixed-term, part-time basis to support recovery for people living with mental ill health. The role covers Warrnambool, Camperdown, Portland and Hamilton within the MDT, offering non-clinical care and practical psychosocial support.

The position is 30.2 hours per week and fixed-term until 30 June 2028. You will work with clients, families and carers to navigate services, strengthen community connections and help achieve

Qualifications

• Minimum Certificate IV in Mental Health or related field.
• Experience working with people with mental health issues, including suicidal ideations and complex needs.
• Ability to build strong relationships with service providers and stakeholders.

Responsibilities

• Provide non-clinical care and practical psychosocial support to clients, families and carers.
• Help clients navigate services and maintain continuity of care.
• Coordinate with multidisciplinary teams and refer to relevant services as needed.
• Travel across Warrnambool, Camperdown, Portland and Hamilton to support communities.
