Better Health Network, Bentleigh East, Victoria, is seeking an experienced Care Coordinator to provide recovery-focused care coordination and individual support within our Mental Health Integrated Complex Care program. The role supports people living with severe mental illness and complex needs, working within a multidisciplinary team to promote recovery and community participation.

You will undertake holistic assessments, develop care plans, coordinate reviews, and build strong partnerships

Qualifications

• Professional qualification in allied health and eligibility for AASW membership.
• Experience providing recovery-focused rehabilitation, care coordination or individual support to people with psychosocial disability or mental illness.
• Experience working in hospital or community mental health settings.
• Ability to work autonomously with sound professional judgement and seek supervision when required.
• Strong communication and relationship-building skills across multidisciplinary teams.
• Understanding social determinants of health with culturally safe practice.
• Maintain safe professional boundaries and apply risk assessment and safety planning.
• Commitment to the principles of the MARAM framework, reflective practice and continuous improvement.
• Current full or probationary driver licence.

Responsabilités

• Completing holistic assessments and developing recovery-oriented care plans.
• Maintaining a caseload and providing one-to-one case work, outreach and discipline-specific support.
• Helping consumers identify achievable goals and access wellbeing, community participation and employment opportunities.
• Coordinating care reviews, case discussions, risk assessments and safety planning.
• Working collaboratively with consumers, families, carers and significant others using family-sensitive approaches.
• Managing referral enquiries and supporting timely entry into the program.
• Maintaining accurate and timely consumer records and contributing to program planning, evaluation and quality improvement.
• Building effective relationships with primary care, tertiary services and other community stakeholders.
