Job summary

Opal HealthCare Forest Lake Care Community is seeking a motivated Registered Nurse to join as Care Coordinator on a permanent full-time basis, 38 hours per week. You will lead clinical management, care planning and educator duties within a supportive team.

Applicants must hold current AHPRA registration, with at least 2 years of RN experience and AN-ACC exposure. The role offers career development and flexible work options within a respected aged care provider.

Qualifications

• Current Australian Registration as a Registered Nurse (AHPRA).
• Minimum of 2 years’ experience as a Registered Nurse.
• Strong clinical, care planning and AN-ACC experience.
• Experience as an Educator highly regarded.

• Coordinate care planning and clinical management as Care Coordinator.
• Provide education and mentoring to team members and staff.
• Lead quality improvement and engagement activities within the community.
