Job summary

Respect is seeking an experienced RN to step into a clinical leadership role as Care Coordinator at Killean Residential Aged Care in Inverell, NSW. You will lead care planning, collaborate with a multidisciplinary team and support residents and families.

With a Bachelor of Nursing and current AHPRA registration, you will drive high-quality, person-centred care, mentor staff and ensure compliance with accreditation standards and quality improvement programs.

Qualifications

• Bachelor of Nursing and current AHPRA RN registration.
• Experience in residential aged care.
• Strong clinical assessment and documentation skills.
• Experience leading multidisciplinary teams.
• Excellent communication and interpersonal skills.
• Confidence with computer systems.

Responsibilities

• Coordinate and review person-centred resident care plans.
• Complete clinical assessments and respond to changes in residents’ needs.
• Support admissions and ensure new residents are appropriately assessed.
• Collaborate with residents, families and healthcare professionals.
• Maintain high standards of clinical documentation.
• Provide and support direct nursing care when required.
• Monitor wounds, falls, clinical deterioration and other clinical risks.
• Support medication management, infection control and clinical audits.
• Assist with investigation and close-out of clinical incidents.
• Guide, support and develop nurses and care staff.
• Support staff orientation, competency assessment and education.
• Promote best-practice care, continuous improvement and strong clinical governance.

Are you an experienced Registered Nurse ready to take the next step into a clinical leadership role? Respect is seeking a passionate and experienced Care Coordinator to join our team at Killean Residential Aged Care in Inverell, New South Wales.

About The Opportunity:

This is an opportunity to play a key role in coordinating high-quality, person-centred clinical care while supporting residents, families and our multidisciplinary care team.

Working closely with the Care Manager and reporting to the General Manager, you will help ensure each resident has a current, individualised care plan that reflects their clinical needs, preferences and goals.

About The Role:

As Care Coordinator, you will have a strong clinical presence within the home and work collaboratively with the broader care team to support safe, consistent and high-quality resident outcomes.

Your responsibilities will include:

• Coordinating and reviewing person-centred resident care plans
• Completing clinical assessments and responding to changes in residents' needs
• Supporting admissions and ensuring new residents are appropriately assessed
• Working closely with residents, families and healthcare professionals
• Maintaining high standards of clinical documentation

About You:

We are looking for an RN who can combine strong clinical knowledge with genuine leadership and a commitment to person-centred care. You will be someone who can confidently engage and support staff, collaborate with residents and families, contribute to continuous improvement and help ensure our clinical practices meet professional standards and legislative requirements.

To be successful in this role, you will have:

• Bachelor of Nursing
• Current AHPRA registration as a Registered Nurse
• Experience working within residential aged care
• Strong clinical assessment, documentation and nursing practice skills
• Experience working collaboratively within multidisciplinary teams
• Excellent communication and interpersonal skills
• Confidence using Microsoft applications and clinical information systems.
• Experience in a similar clinical leadership role, knowledge of aged care accreditation and quality improvement programs, and a current driver's licence will be highly regarded.
