Job summary

The District Nurses in Tasmania are seeking a Care Coordinator who is passionate about helping people stay safely at home. You will assess needs, develop care plans and coordinate services with a multidisciplinary team to achieve optimal client outcomes.

Applicants should have a current Australian driver’s licence, be prepared for police checks and a Working with Vulnerable People/Children check, and hold a minimum Certificate III.

Qualifications

• Current Australian driver's licence.
• National police check.
• Working with vulnerable people/children check.
• Minimum Certificate III in a relevant discipline.

• Conduct assessments: including identifying any health risks, medical conditions, or social and environmental factors
