Responsibilities

• Provide administrative and coordination support to multiple Care Partners across a shared client portfolio.
• Prepare Service Agreements, variation letters and other client documentation.
• Prepare and update individualised client budgets.
• Assist with updating care plans as directed by the Care Partner.
• Liaise with internal departments, clients, representatives and service providers to support timely coordination and delivery of services.
• Raise and document incidents, hazards and client feedback within organisational systems, ensuring timely escalation where required.
