Job summary

VMCH is seeking a Care Partner based in Pakenham to support older Australians across Eastern and Southern Metropolitan Melbourne, with regular travel across metropolitan and regional areas. You will coordinate services, develop care plans, manage budgets and ensure quality care in clients’ homes.

The role emphasizes person-centred care, empowering clients and carers, and working within a multidisciplinary team to meet KPIs and service targets.

Qualifications

  • Diploma qualification or higher in a relevant field (e.g. Diploma, Advanced Diploma, Bachelor degree or above).
  • Experience in case management, care coordination or client services within aged care or community services.
  • Experience completing assessments, developing care plans and supporting clients with complex needs.
  • Ability to manage consumer budgets and support services effectively.
  • Sound computer skills including Microsoft Office and client management systems.
  • Excellent organisational skills with the ability to prioritise and work autonomously.
  • Understanding of community aged care services, referral pathways and person-centred care principles.
  • Knowledge of aged care legislation, quality standards and sector reforms.
  • Current Victorian Driver's Licence and car registration.
  • Willingness to travel across Metropolitan & Regional areas.
  • National Police Check

  • Deliver high-quality, person-centred care and support to clients in their homes and communities.
  • Conduct comprehensive assessments, develop individualised care plans and coordinate services.
  • Empower clients and carers to make informed choices about their care and support.
  • Manage your own client portfolio, including budgets, services and changing care needs.
  • Ensure services are delivered in line with aged care quality standards and organisational policies.
  • Maintain accurate case notes, documentation and client confidentiality.
  • Work collaboratively within a supportive, multidisciplinary team and escalate concerns where required.
  • Work towards agreed KPIs and service delivery targets, balancing quality client outcomes with productivity and service growth.

The position

As a Care Partner, you are at the heart of our mission, empowering older adults to remain independent, safe and connected within their own homes and communities.

Based in Pakenham, you’ll support clients across Eastern and Southern Metropolitan Melbourne, extending through to Gippsland. Regular travel is an important part of the role, allowing you to build meaningful relationships with clients and coordinate personalised services that enhance their wellbeing, independence and quality of life.
