Job summary

Social Futures Ltd in Gosford, NSW, is seeking a Support Coordinator to join our Universal Aftercare program – Care Connect on a full-time basis. You will work with participants to identify needs, set goals and connect them with health, clinical and community services to support wellbeing.

We require a Diploma or Cert IV in Mental Health or Community Services and a minimum of two years' experience in suicide prevention, aftercare or mental health settings, with a strong focus on person-centred

Qualifications

• Experience providing person-centred support, assessment and case management.
• Minimum two years’ experience in suicide prevention, aftercare or mental health settings.
• Qualification at Diploma or Cert IV level in Mental Health or Community Services.

Responsibilities

• Work collaboratively with participants to identify needs, set goals and develop safety and care plans.
• Assist participants to connect with health, clinical and community-based support services.
• Support the planning and implementation of suicide prevention activities with participants.
