Job summary

You will assess client needs, develop tailored plans, and work collaboratively with health professionals and government agencies to promote wellbeing, independence, and social inclusion.

Qualifications

• Qualification in Community Services, Social Work or equivalent.
• Experience in a community services or support role.
• Strong communication, interpersonal and organizational skills.
• Ability to work independently and as part of a team.
• Knowledge of community support systems and referral pathways.

Responsibilities

• Assess client needs and connect them with health, housing, employment, education, and social services.
• Interview clients to identify challenges and develop support plans.
• Plan and deliver education, training, and support programs to promote wellbeing.
• Collaborate with families, health professionals, and government agencies.
• Refer clients to service providers and follow up for continuity of care.
• Monitor progress and evaluate programs, recommending improvements.
• Maintain confidential client records and reports.
• Promote social inclusion, equity, and respect for diversity.
• Advocate for clients' rights and community participation.
• Support clients in developing life skills and independence.
