Job summary

Outcomes Connect Australia is seeking a Behaviour Support Practitioner to support NDIS participants and their families, focusing on improving quality of life and reducing behaviours of concern.

You will work with a team of practitioners, a team leader and clinical advisors to provide evidence-based practice, while developing your own knowledge and skills. Flexible scheduling and remote/community-based work are offered in Australia.

Qualifications

• Current suitability (i.e. registration) with the NDIS Commission as a Behaviour Support Practitioner is preferred but not essential.
• Considerable practical experience in supporting people with complex and challenging behaviours is essential.
• Demonstrated ability to work positively in a diverse team.
• Outstanding communication, influencing and interpersonal skills.
• Flawless writing skills.

Responsibilities

• Support our clients (NDIS participants), along with their families and/or carers, to identify their needs through assessment and person-centred planning.
• Identify meaningful goals and practical strategies utilizing The Positive Behaviour Support Capability Framework, with a commitment to eliminate restrictive practices, in the home and community.
• Develop, deliver and evaluate evidence-based support plans within a complex support space.
• Ensure individual plans reflect our clients' goals, passions and strengths.
• Build the knowledge, capacity and strengths of those who support our clients - family, carers, friends, etc.
