Job summary

Outcomes Connect Australia is seeking a Behaviour Support Practitioner to support people living with disability. You’ll focus on improving quality of life, reducing challenging behaviours and promoting rights.

You’ll work with a team of practitioners, a team leader and clinical advisors in a community-based setting near Warrnambool. You’ll engage in evidence-based practice, attend PBS training with the School of PBS and MedHealth Academy, and contribute to a supportive, values-led culture

Qualifications

• Degree-level qualifications in relevant field preferred.
• NDIS Commission suitability preferred but not essential.
• Experience supporting people with complex behaviours desirable.

Responsibilities

• Assess and plan with clients (NDIS participants) and families.
• Set meaningful goals and practical strategies using PBS framework.
• Develop, deliver and evaluate evidence-based support plans.
• Ensure plans reflect clients' goals and strengths.
• Build capacity of family, carers and support network.
