Job summary

Ability Action Australia is seeking Behaviour Support Practitioners of all experience levels in Ryde, Bankstown and surrounding areas. You will assess participants, develop and deliver evidence-based support plans, and work within a multidisciplinary team to achieve meaningful, person-centred outcomes.

Qualifications

• A degree qualification as an allied health professional.
• Experience in report writing.
• Strong organisational skills and the ability to manage a varied caseload.
• A proactive, self-motivated approach, with a drive to improve outcomes and test new ideas.
• A genuine passion for working in the disability sector.

Responsibilities

• Supporting NDIS participants and their families to identify needs through assessment and person-centred planning.
• Owning assessment, development and delivery of evidence-based support plans in a complex environment.
• Driving meaningful goals and measurable outcomes with a positive behaviour support framework.
• Ensuring plans reflect each participant’s goals, strengths and what matters most.
• Building the capability and confidence of those supporting participants.
• Collaborating within a multidisciplinary team and mentoring less experienced practitioners.
